If you have been advised by the surgery to submit a smoking review on a regular basis please use the Smoking Review form.
Chronic Disease Clinics
Clinics are available for patients with Coronary Heart Disease, Diabetes Mellitus, Asthma and COPD.
If you have been advised by the surgery to submit a COPD assessment please use the COPD Assessment form.
If you have been advised by the surgery to submit an annual review of your asthma symptoms please use the Asthma Review form. If your symptoms are deteriorating or you are having any concerns please make an appointment with our Nurse.
Health Checks
If you are aged between 16 and 74 years and have not attended the surgery in the last three years, you may request an appointment for a health check. If you are 75 years and over, this applies if you have not attended for 12 months.
Sick Certification
A sick certificate from the doctor only becomes necessary if an illness lasts more than six days. Shorter illnesses are covered by self certification.

Minor Injuries
We aim to help you and your family get the care you need as close to home as possible and we are always looking for ways of improving our service. With this in mind we are pleased to announce that we are now able to treat many minor injuries at the surgery.
When Should You Call The Surgery?
Call the surgery when you or someone you care for sustains a minor injury that is outside the usual level of simple domestic first aid that you can provide or you require further advice or guidance.
What Types Of Injuries Can Be Treated At The Surgery?
- Sprains to hands, arms, legs, feet
- Minor bruises
- Minor burns – scalds and blisters
- Lacerations and minor head Injuries – bump to your head, cuts, wounds and scratches
- Minor eye injuries

When you call us the reception staff will give you an appointment time or advise what to do if an alternative arrangement would suit your needs better.
